NOTARIZATION OF AFFIDAVIT

Once you recieve the Affidavit, you will then take a print out of it and get it notarised from a local notary

DISPATCHING DOCUMENTS

After the successful completion of the notarization process and will simple have to take the documents and courier it to us

SUBMISSION OF APPLICATION

The application along with all documents will be submitted at the office of the Registrar of Births and Deaths

COLLECTING AND DISPATCHING DEATH CERTIFICATE

After issuance of death certificate, which could take up to 2-3 weeks after submission of your application, we will collect and dispatch the same to you through courier

Documents required for Death Certificate

The following documents are required to obtain a death certificate:

  • Proof of Birth of the deceased

    • Photocopy of any one of the following – birth certificate, matriculation certificate, aadhaar card, PAN card, driving license, voter id card
  • Affidavit specifying the date and time of death

  • Identity Proof of the deceased person

    • Photocopy of any one of the following documents will suffice: aadhar card, passport, voter ID card, PAN card, driving license
  • Proof of Death

    • Photocopy of any of the following – crematorium receipt, hospital letter etc.
  • Passport size photo of the deceased

  • Signed authority letter
