Birth Certificate
Overview

A Birth Certificate is the most important identity document and serves to establish the date and fact of one’s birth for a whole range of purposes, like acquiring the right to vote, admission to schools, obtaining Government identity documents like a driving license or passport. MyAdvo will take care of all the documentation and formalities to get the birth certificate.

 

Birth certificate may be required in one of the following instances:

  • New born
  • Misplacement of original birth certificate
  • Birth certificate was never issued
Steps involved in obtaining Birth Certificate
Documents and Questionnaire
A checklist of documents/questionnaire will be provided to record your details.
Drafting
Upon receiving filled questionnaire, we will draft an Affidavit and Authority Letter. We will send you soft copies of these documents for signatures.
Dispatch of Documents
You will be required to send the documents to us through courier or any other means.
Submission of Application
The application along with all documents will be submitted at the office of the Registrar of Births.
Collection and dispatch of Birth Certificate
After issuance of birth certificate, which could take 20-30 days after submission of application, we will collect the certificate and send it to you through courier.
Required Documents
  • Affidavit of one parent/ relative specifying place, date and time of birth of the person
  • Proof for date of birth (photocopy of any one of the following - class 10th certificate, school leaving certificate, Aadhar card, voter ID card, PAN card, driving license)
  • Identity proof of one Parent (photocopy of any one of the following documents will suffice: Aadhar card, passport, voter ID card, PAN card, driving license)
  • Photocopy of identity proof of the Applicant (photocopy of any one of the following documents will suffice: Aadhar card, passport, voter ID card, PAN card, driving license)
  • Proof of address where the birth of a child took place (photocopy of any one of the following documents will suffice: house tax receipt, allotment letter, sale deed, hospital record)
  • Signed authority letter
